What we collect
- Consult requests. When you use the "Have us reach out" form, we receive your name, email, phone number (if given), your child's age, your town, your preferred setting and time to talk, how you heard about us, and any message you write.
- Online booking. When you pick a consult time, Google Calendar collects the name, email and other details you enter on the booking page.
- Intake form. Before the consult we ask you to complete a short intake form through Google Forms about you, your child and your speech and language concerns.
- Messages. Anything you send us by email, phone or text.
Please don't include detailed medical information in the website request form or in email. Jenelle will go over everything with you during the consult.
How we use it
- To reply to you and schedule your free consult
- To prepare for the consult and, if you choose to work with us, to provide evaluation and therapy
- To send invoices and monthly superbills
- To understand how families find us, for example whether a pediatrician or school referred you
What we never do
- We never sell or rent your information.
- We don't use your information for advertising, and our website doesn't run advertising trackers.
- We don't share anything about your child with a school, daycare, teacher or other provider without your written permission.
Children's information
Information about a child comes from their parent or guardian. Our website is meant for parents and caregivers, and we don't knowingly collect information directly from children.
How we protect it
Protecting your family's information is part of caring for your child. All information you share with Shining Voices is handled in compliance with HIPAA, the federal law that protects the privacy and security of health information.
- Information is kept in secure, password-protected business accounts.
- Access is limited to the people who run the practice.
- We keep information only as long as we need it.
- We share information only as described here, when the law requires it, or to protect a child's safety.
Once your child becomes a client, you'll receive our Notice of Privacy Practices, which explains your rights under HIPAA in detail.
Your choices
You can ask to see, correct or delete the information you've given us at any time. If you reach out but decide not to work with us, just let us know and we'll delete your request and intake form.
